and present the screen of submitted form for reference to the reception

Taiwan Centers for Disease Control (CDC) has announced that
inbound travelers have to be self-quarantined for 14 days.
Additionally, travelers tested negative for Covid-19
still have to be self-managed of health for another 7 days.
We apologize for not able to provide service for
(I) Travelers currently under self-quarantine and/or self-health management
(II) Travelers who were in contact with persons
under self-quarantine and/or self-health management
within 21 days, until the policy is lifted by CDC.
